Roles & Responsibilities

• Provide compassionate, patient-centered nursing care following facility policies and LPN scope of practice
• Administer medications, document accurately, and monitor patients for response and side effects
• Collaborate with RNs, providers, and interdisciplinary teams to support care plans and patient outcomes
• Perform routine assessments, report changes in condition, and escalate concerns promptly
• Maintain accurate, timely documentation in the patient record and support care transitions
• Support infection control, safety protocols, and a positive environment for patients and families

Qualifications

• Current LPN license (or eligible to obtain licensure) — required
• Current BLS/CPR certification at time of hire (or ability to obtain prior to start)
• Clinical experience caring for adults in long-term care or rehabilitation settings preferred
• Strong communication skills and the ability to work collaboratively on a multidisciplinary team
• Ability to safely perform duties for a 12+ hour day shift and maintain accurate documentation

Education

• Practical Nursing diploma or certificate (LPN program)

Knowledge & Skills

• Solid working knowledge of medication administration and documentation best practices
• Skilled at bedside assessments, wound care basics, and monitoring vital signs
• Comfortable with electronic health record systems and charting
• Ability to prioritize tasks in a fast-paced clinical environment
• Compassionate patient communication and family education skills
