Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at Laconia Rehab Ctr
• Administer medications and treatments accurately and safely
• Collaborate with the interdisciplinary team to implement individualized care plans
• Perform regular patient assessments and document changes in condition
• Support restorative and rehabilitative efforts to help residents regain independence
• Communicate clearly with families and team members to ensure continuity of care
• Maintain a safe, clean environment and adhere to facility policies and infection control

Qualifications

• Current LPN license in New Hampshire or eligibility to obtain state licensure
• Completion of an accredited Practical/Vocational Nursing program
• Current BLS/CPR certification (or ability to obtain prior to start)
• Reliable clinical skills in medication administration, wound care, and resident assessment
• Ability to pass required background checks and health screenings
• Strong communication, teamwork, and time-management skills

Education

• Graduate of an accredited Practical/Vocational Nursing program
• Current LPN license required

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and dressing changes
• Vital sign monitoring and patient assessment
• Familiarity with long-term care and rehab nursing practices
• Comfortable using electronic health records and charting
• Excellent interpersonal skills and resident-focused care
