Roles & Responsibilities

• Evaluate patients to determine baseline function and individualized goals
• Develop and implement evidence-based treatment plans to restore mobility and function
• Provide hands-on therapy and therapeutic exercise instruction with clear progression
• Document patient progress accurately and communicate updates to the care team
• Collaborate with physicians, nurses, and support staff to optimize patient outcomes
• Educate patients and families on home programs and injury prevention to support long-term success
• Participate in team meetings and contribute to continuous quality improvement

Qualifications

• Active Physical Therapist license in Illinois (or ability to obtain prior to start)
• Current BLS/CPR certification (or willingness to obtain)
• Experience in outpatient physical therapy preferred
• Strong clinical reasoning and patient communication skills
• Ability to work full-time for the duration of the contract (2025-12-29 to 2026-03-23)

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in physical therapy assessment and treatment planning
• Knowledge of therapeutic exercise progression and manual therapy techniques
• Proficient documentation and electronic medical record usage
• Excellent interpersonal skills and team collaboration
• Ability to educate patients and caregivers clearly and compassionately
