Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across greater Sacramento county
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain mobility and independence with personalized treatment plans
Collaborate with interdisciplinary teams to deliver exceptional home health care
Document patient progress and maintain accurate clinical records

Qualifications

Active Physical Therapy Assistant license in California
Prior home health experience preferred
New graduates considered in exceptional cases only
Strong communication and interpersonal skills
Ability to work independently in various home settings

Knowledge & Skills

Experience with diverse patient populations and diagnoses
Knowledge of home health documentation requirements
Comfortable traveling between patient homes
Strong organizational and time management abilities
Compassionate bedside manner with geriatric patients
