Location: Louisville, Kentucky

Start Date: 06-22-2026

End Date: 09-21-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide essential home health physical therapy services to patients recovering from CHF, orthopedic procedures, and pneumonia
Conduct start of care assessments, recertifications, and evaluations that directly impact patient recovery
Deliver meaningful daily visits that help patients regain strength and independence in their own homes
Collaborate with clinical managers and educators to ensure the highest quality care
Make a real difference in patients' lives by bringing expert therapy directly to where they feel most comfortable

Qualifications

Current Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but new grads welcome to apply
Strong communication skills and compassion for patient-centered care

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and evaluation skills
Knowledge of CHF, orthopedic, and pneumonia recovery protocols
Ability to work independently in various home settings
Strong documentation and time management abilities
