Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes
Help patients recover from surgeries, falls, and other conditions with personalized care plans
Collaborate with home health and hospice teams to deliver exceptional patient outcomes
Travel within a 60-mile radius to bring quality care directly to patients
Document patient progress using HomeCare/HomeBase EMR system

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
1+ year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related clinical skills

Knowledge & Skills

Physical Therapy expertise in home health settings
HomeCare/HomeBase EMR proficiency
Ability to manage 5-7 patients per day independently
Comfortable working with home health and hospice populations
Excellent time management and organizational skills
