Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes throughout Yolo County
Assess and evaluate each patient's therapeutic needs while developing comprehensive care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Monitor progress and communicate changes to physicians to optimize treatment outcomes
Train patients in prosthetic device use and coordinate with home health aides
Document clinical progress and participate in case conferences with care teams
Identify additional home health service needs and make appropriate referrals

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid Physical Therapy license for California practice
Ability to work independently while thriving in collaborative care environments

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient and family education for rehabilitation success
Understanding of Medicare and Medi-Cal regulations
Prosthetic device training and adaptive equipment knowledge
Strong documentation and clinical reporting abilities
