Roles & Responsibilities
- Conduct OASIS assessments to develop personalized care plans
- Provide Start of Care visits that set patients up for success
- Help elderly patients recover from falls and fractures with compassion and expertise
- Manage a manageable caseload of 4-6 patient visits each day
- Document visits accurately using tablets for seamless tracking
Qualifications
- Licensed Physical Therapist in Indiana
- Experience with OASIS assessments and Start of Care visits
- Comfortable working independently in rural settings
- Strong communication and time-management skills
