Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes throughout Yolo County
Assess and evaluate each patient's therapeutic needs while developing personalized care plans
Guide patients and families through rehabilitation exercises and therapeutic appliance use
Collaborate with physicians and care teams to track progress and adjust treatment goals
Train and supervise home health aides participating in patient rehabilitation
Document clinical progress and communicate changes in patient condition to physicians
Identify additional home health service needs and coordinate appropriate referrals

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organization abilities
Valid driver's license and reliable transportation for home visits across Yolo County
Two supervisory references from your specialty area (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment techniques
Patient and family education on therapeutic exercises and equipment
Understanding of Medicare and Medi-Cal regulations
Ability to work independently while maintaining strong team communication
Proficiency in documenting clinical progress and patient outcomes
