Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health and hospice patients
Help patients recover from surgeries, falls, and other common conditions
Travel within a 60-mile radius to deliver personalized care in patients' homes
Document visits accurately using HomeCare/HomeBase EMR
Collaborate with interdisciplinary teams to support patient goals
Educate patients and families on safe mobility and exercise techniques

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed
At least 1 year of physical therapy experience
Previous home health experience strongly preferred
Valid driver's license and auto insurance required
Comfortable working independently without on-site manager supervision

Knowledge & Skills

Home health physical therapy expertise
Familiarity with HomeCare/HomeBase EMR system
Strong assessment and clinical decision-making skills
Excellent communication and patient education abilities
Ability to manage a 5-7 patient daily caseload efficiently
