Roles & Responsibilities

Deliver personalized physical therapy care directly in patients' homes
Create customized treatment plans that help patients regain strength and mobility
Collaborate with families and healthcare teams to support recovery goals
Document progress and adjust care plans to meet individual needs

Qualifications

Active California Physical Therapist license
BLS certification
Home health experience preferred
Valid driver's license and reliable transportation

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and care planning
Strong communication and documentation skills
Ability to work independently in various home settings
