Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Conduct comprehensive evaluations, develop treatment plans, and implement interventions
Perform start of care, recertifications, and resumption of care visits
Educate patients and families on home exercise programs and safety
Collaborate with the interdisciplinary team to ensure coordinated care
Complete thorough OASIS documentation and maintain compliance with regulations

Qualifications

Active Physical Therapy license in North Dakota (or compact eligibility)
BLS certification from the American Heart Association
Valid driver's license and auto insurance
Minimum 1 year of home health experience preferred
OASIS training and experience highly desired
Comfortable with a productivity expectation of 6.5 points per day

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent time management and ability to work independently
Proficiency in OASIS documentation and home health regulations
Effective communication with patients, families, and healthcare team
Competence in using EMR systems and telehealth platforms
