End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Perform comprehensive evaluations, develop treatment plans, and implement interventions
Conduct start of care visits, recertifications, and discharges as needed
Educate patients and families on home exercise programs and safety
Collaborate with a multidisciplinary team to ensure optimal patient outcomes
Document patient progress accurately and timely using provided tablet

Qualifications

Active Physical Therapist license in Kentucky (or compact eligibility)
Current BLS certification
Prefer at least 1 year of experience, but new grads may be considered
Home health experience is a plus
Valid driver's license and reliable transportation

Education

Graduate from an accredited Physical Therapy program

Knowledge & Skills

Expertise in evaluating and treating orthopedic, CHF, and pneumonia patients
Ability to work independently in a home health setting
Excellent communication and patient education skills
Proficient with electronic documentation and telehealth tools
