Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide physical therapy services to patients in their homes according to a physician's plan of care
Assess and evaluate functional status, muscle strength, mobility, gait, and range of motion
Develop and update therapeutic plans, setting goals and tracking progress
Instruct patients and families on therapeutic exercises and use of assistive devices
Supervise home health aides and homemakers participating in patient care
Document clinical notes, summaries, and patient responses in health records
Collaborate with physicians, case conferences, and other healthcare team members
Identify patient needs for additional home health services and make referrals

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute care, home health, or rehab setting
Active BLS certification required
Strong communication and organizational skills
Willingness to travel within Yolo County service area
Acceptable dress code: scrubs or slacks with polo/button-up shirt

Knowledge & Skills

Expertise in physical therapy assessment and treatment planning
Proficiency with home health regulations, including Medicare Conditions of Participation and Title 22
Excellent patient and family education abilities
Skilled in documentation and clinical record keeping
