Registered Nurse - Case Management
Location: Carmichael, California

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; rotating weekends (at least one full weekend per month)
Hours per Week: 40
Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Conduct comprehensive initial assessments, utilization review, and discharge planning for a caseload of 15-20 patients daily
- Collaborate with interdisciplinary teams to coordinate care across all hospital areas (no outpatient or home care)
- Apply Interqual criteria for utilization management and ensure appropriate resource utilization
- Work with diverse patient populations including uninsured and homeless individuals
- Document all care coordination activities in Cerner EMR
- Participate in rotating weekend coverage (at least one full weekend per month)
Qualifications
- Current and active California RN license
- BLS certification from American Heart Association
- Prior recent experience at a Dignity Health/CommonSpirit facility in California (required)
- Minimum 1 year of case management experience, preferably with utilization review and discharge planning
- Experience with Interqual criteria and Cerner EMR preferred
- Ability to work rotating weekends and adapt to reprioritization throughout the day

- Effective communication and teamwork
- Ability to reprioritize in a fast-paced environment
- Proficiency with Interqual and Cerner (preferred)
