Roles & Responsibilities

• Process and manage insurance authorizations to keep care moving smoothly
• Coordinate post-acute referrals and arrange transitions to home care or facilities
• Deliver Medicaid notices to patients with accuracy and compassion
• Document thoroughly and efficiently in Epic to support continuity of care
• Work with internal and external team members to arrange DME and follow-up services
• Refer patients to rehab and long-term care resources when appropriate
• Support discharge planning by assessing needs and connecting resources
• Educate patients and caregivers to help them navigate post-discharge plans
• Participate in pre-cert review and utilization tasks to ensure appropriate care pathways

Qualifications

• Minimum 1 year of relevant experience (1 year required)
• Epic charting experience is required — you’ll be using Epic daily
• MSW preferred but not required — clinical background helpful, not mandatory
• Washington state license is not required for this role
• First-timers accepted — if this is your first assignment, we’ll welcome you
• Travel experience is not required; locals are not accepted for this posting
• Completion of the 40-hour orientation is required

• MSW preferred

Knowledge & Skills

• Solid understanding of care coordination and discharge planning
• Experience with Medicaid and Medicare processes and community programs
• Prior authorization and pre-certification workflow knowledge
• Familiarity ordering/assessing DME needs and benefits eligibility
• Strong documentation skills and attention to detail in Epic
• Ability to coordinate with interdisciplinary teams and external partners
• Comfort educating patients and caregivers about post-discharge plans
