Contract Type: Per-Diem

Shift Type: 8-hour shift (exact start/end times to be assigned by facility)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and monitor resident condition
Administer medications and treatments accurately and timely
Document assessments, interventions and responses in the medical record
Collaborate with the interdisciplinary team to support individualized care plans
Provide clear communication to residents and families about care needs
Assist with admissions, discharges and transitions to ensure continuity of care
Maintain infection control and safety standards at all times

Qualifications

Current LPN license (licensed practical nurse) in good standing
Active nursing license in Pennsylvania or compact/reciprocal eligibility
Basic Life Support (BLS) certification required or able to obtain
Prior experience in skilled nursing or long-term care preferred
Strong interpersonal skills and ability to work as part of a multidisciplinary team

Education

LPN diploma or equivalent from an accredited nursing program

Knowledge & Skills

Skilled in medication administration and wound care basics
Proficient with clinical documentation and care planning
Knowledgeable about infection control and resident safety practices
Good clinical assessment skills and the ability to escalate concerns appropriately
Effective communication with residents, families and care team members
