Contract Type: Per-Diem

Shift Type: 8-hour shifts (exact start/end times and days to be confirmed by facility scheduling)

Hours per Week: 40

Candidate Pay Rate: $38 per hour

Roles & Responsibilities

Provide direct resident care with compassion and professionalism
Administer medications and treatments accurately and safely
Monitor and document resident condition and communicate changes promptly
Assist in care planning and follow established care plans
Collaborate with RNs, CNAs, and interdisciplinary team members to deliver coordinated care
Support residents with activities of daily living and promote independence
Maintain accurate and timely clinical documentation in resident records
Foster positive relationships with residents and families through clear communication

Qualifications

Current LPN license in Connecticut (required)
Graduation from an accredited LPN program
Basic Life Support (BLS) / CPR certification (or willingness to obtain)
Comfort working in a skilled nursing/long-term care environment
Strong communication and teamwork skills
Clinical experience in nursing care preferred (experience in long-term care a plus)

Education

Completion of an accredited LPN/Vocational Nursing program (diploma or certificate)

Knowledge & Skills

Medication administration and documentation
Basic patient assessment and vital signs monitoring
Wound care fundamentals and infection control practices
Familiarity with electronic medical records / clinical charting
Time management, prioritization, and delegation within the care team
Compassionate resident-centered care and strong interpersonal skills
