Contract Type: Per-Diem

Shift Type: 8-hour shift; specific start/end times and days to be confirmed

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, resident-centered nursing care in a long term care setting
Administer medications and treatments accurately and on time
Monitor resident conditions, document changes and communicate effectively with the care team
Support admissions, discharges and care transitions with professionalism
Collaborate with nurses, therapists and support staff to implement individualized care plans
Maintain a safe, clean and respectful environment for residents and families

Qualifications

Current LPN/LVN license in Pennsylvania or compact state if applicable
Completed an approved practical nursing program
Basic Life Support (BLS) certification required
Minimum clinical experience in long term care or similar preferred
Strong communication and documentation skills
Ability to pass facility background checks and health screenings

Education

Practical nursing diploma or certificate from an accredited program
High school diploma or equivalent

Knowledge & Skills

Medication administration and wound care fundamentals
Familiarity with long term care workflows and regulatory standards
Electronic medical record documentation skills
Ability to assess changes in resident condition and escalate appropriately
Teamwork and interprofessional communication
Time management and prioritization under a busy schedule
