Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents
• Administer medications and treatments accurately and on schedule
• Monitor, record and report changes in patient condition promptly
• Collaborate with nurses, care staff and leadership to implement care plans
• Assist residents with activities of daily living with dignity and respect
• Maintain accurate, timely documentation in clinical records
• Support infection control and safety procedures to protect residents and staff
• Participate in shift handoffs and team communication to ensure continuity of care

Qualifications

• Current LPN (Licensed Practical Nurse) license in Connecticut (required)
• BLS/CPR certification from an accredited provider (required or must obtain upon hire)
• Professional license in good standing with no active restrictions
• Comfortable with medication administration, charting and basic patient assessments
• Excellent communication and interpersonal skills with residents and families
• Ability to pass background check and drug screening (facility policy)
• Previous long-term care or skilled nursing experience preferred

Education

• Graduate of an accredited practical nursing program (diploma or certificate)

Knowledge & Skills

• Knowledge of safe medication administration and documentation practices
• Familiarity with basic wound care, vital sign monitoring and IV/PO routes as applicable
• Understanding of infection control standards and resident safety
• Strong clinical judgment and the ability to prioritize tasks under pressure
• Compassionate bedside manner and teamwork-oriented attitude
• Competence with electronic health records or clinical charting systems
