Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to optimize functional outcomes
• Develop and implement individualized treatment plans that help residents reach their highest potential
• Collaborate with an interdisciplinary team to coordinate care and ensure continuity
• Document accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and contribute to ongoing quality improvement efforts

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical skills with demonstrated ability in assessment and treatment planning
• Excellent communication and interdisciplinary collaboration skills
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented attitude with commitment to compassionate care
