Roles & Responsibilities

• Assist therapists in delivering skilled physical therapy interventions to SNF residents
• Implement treatment plans and document progress accurately in the clinical record
• Collaborate with the interdisciplinary team to support resident care goals
• Educate residents and families on home exercise programs and safe mobility techniques
• Monitor and report changes in patient status to the supervising therapist
• Support efficient clinic flow while maintaining high standards of safety and compassion

Qualifications

• Current PTA license in Indiana or eligibility to obtain state licensure
• Graduate of an accredited Physical Therapy Assistant program
• New grads welcome — the client accepts clinicians with any level of experience
• Active BLS/CPR certification (or willingness to obtain upon hire)
• Ability to pass required background checks and facility screenings
• Strong interpersonal skills and commitment to patient-centered care

Education

• Graduate of an accredited Physical Therapy Assistant (PTA) program — typically an Associate degree

Knowledge & Skills

• Familiarity with SNF (skilled nursing facility) therapeutic practices and documentation
• Proficient in safe manual handling, transfers, gait training, and therapeutic modalities
• Good clinical judgment and ability to follow established care plans
• Clear communication skills for effective teamwork and patient/family education
• Comfortable using electronic health records for documentation and scheduling
