Roles & Responsibilities

• Evaluate and assess residents to create individualized physical therapy plans
• Deliver skilled therapeutic interventions to improve mobility, function, and independence
• Document evaluations, treatment plans, and progress notes accurately and timely
• Collaborate with nurses, OT, SLP, and facility staff to coordinate resident care
• Meet facility productivity expectations and maintain quality outcomes (90% productivity target)
• Adhere to CRG policies and procedures and facility standards, including dress code
• Be present onsite to ensure continuity of care and to receive guaranteed hours

Qualifications

• Licensed Physical Therapist or DPT (PT/DPT required or eligible)
• 1 to 3 years of clinical experience in a skilled nursing or post-acute setting preferred
• Comfort working in SNF practice setting and collaborating with interdisciplinary teams
• Strong documentation skills and ability to meet productivity expectations
• Willingness to follow facility dress code (black scrubs) and CRG policies

Education

• Doctor of Physical Therapy (DPT) preferred; appropriate PT degree/certification required

Knowledge & Skills

• Proven clinical skills in assessment, therapeutic exercise, and mobility training
• Knowledge of SNF therapy protocols and post-acute care workflows
• Ability to develop and implement resident-centered care plans
• Clear communication and teamwork skills with clinical and non-clinical staff
• Time management and documentation accuracy to meet productivity targets
