Roles & Responsibilities

• Administer medications and treatments safely and accurately while following facility protocols
• Monitor patients’ vital signs and clinical status, escalating changes to the care team promptly
• Provide compassionate direct patient care, helping with activities of daily living and comfort measures
• Document patient assessments, interventions, and responses clearly in the medical record
• Collaborate with nurses, providers, and support staff to deliver coordinated care
• Assist with admissions, discharges, and transfers to ensure smooth patient flow
• Support infection prevention and safety practices to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Hampshire (required)
• Current Basic Life Support (BLS) certification or willingness to obtain
• Clinical experience as an LPN preferred; per diem-friendly to returning nurses
• Ability to pass facility-required health screenings and background checks
• Strong communication and teamwork skills; dependable and patient-focused

Education

• Graduation from an accredited practical nursing program (LPN)

Knowledge & Skills

• Proficient in medication administration and documentation
• Comfortable performing wound care, IV care support per facility policy, and wound assessments
• Skilled at taking and interpreting vital signs and focused patient assessments
• Familiarity with electronic medical records and accurate charting
• Good time management and prioritization in fast-paced clinical settings
• Patient education and family communication delivered with empathy and clarity
