Roles & Responsibilities

• Provide comprehensive occupational therapy evaluations and create individualized treatment plans
• Deliver skilled OT interventions focused on improving activities of daily living, mobility, and independence
• Collaborate closely with nurses, PTs, SLPs and the interdisciplinary team to coordinate patient-centered care
• Document progress clearly and timely in the patient record to support quality outcomes
• Educate patients and family members on adaptive techniques, durable medical equipment, and home safety
• Participate in care planning meetings and contribute to discharge planning to ensure safe transitions

Qualifications

• Licensed Occupational Therapist (OT) or OTR credential; active or eligible for South Dakota licensure
• Degree in Occupational Therapy (see Education section)
• Current BLS/CPR certification (or ability to obtain prior to start)
• Strong clinical documentation skills and ability to manage caseload in a skilled nursing/long-term care setting
• Willingness to complete facility onboarding, background check, and required health screenings

Education

• Bachelor’s or Master’s degree in Occupational Therapy (or equivalent professional OT degree)

Knowledge & Skills

• Skilled in ADL training, adaptive equipment recommendations, and functional mobility interventions
• Proficient clinical assessment and treatment planning with measurable goals
• Clear communication and strong teamwork across interdisciplinary teams
• Time management and organizational skills to handle a consistent caseload
• Familiarity with clinical documentation best practices and regulatory standards
