Contract Type: Travel Contract

Shift Type: Monday-Friday (business hours), Day shift. Schedule is 5x8-hour days with shift start options: 7:00 AM - 3:30 PM / 7:30 AM - 4:00 PM / 8:00 AM - 4:30 PM / 8:30 AM - 5:00 PM. Weekend requirement: every other weekend (or 4 weekend days in a 4-week schedule). Float within case management scope to cover gaps as needed.

Roles & Responsibilities

• Coordinate and manage patient care across the continuum by leading discharge planning and transitions to next level of care
• Perform utilization review and apply Milliman guidelines to ensure appropriate, high-quality care decisions
• Conduct needs assessments and arrange DME, durable medical equipment, and community resources as required
• Document thoroughly in Epic to maintain accurate case records and support care team communication
• Collaborate with interdisciplinary teams including social services, PT/OT, pharmacy, respiratory, and radiology to expedite safe discharges
• Advocate for patients and families to create patient-centered plans of care that reduce readmissions and optimize outcomes
• Float within the case management service to cover gaps and ensure continuity of care across units

Qualifications

• Active Alaska RN license in hand required for auto-offer (screening for pending licenses will occur)
• Minimum 1 year acute care nursing experience (first timers accepted)
• Epic EMR experience required (Epic hardstop noted)
• Acute hospital / acute care experience required; behavioral health experience does not apply
• ACM certification preferred
• BLS preferred
• Willingness to work every other weekend and float within case management; flexible team player
• Ability to follow CMS and HIPAA regulations and utilization review best practices

Education

• Registered Nurse (RN) - Active Alaska RN license required

Knowledge & Skills

• Case management and care coordination
• Discharge planning and transition of care processes
• Utilization review and Milliman guidelines
• Epic charting and documentation workflows
• Familiarity with acute care specialties: ICU, medical-surgical, pediatrics, SDU/PCU/IMC/Obs, women’s services
• Microsoft Office and Microsoft Teams for communication and documentation
• Strong interdisciplinary collaboration and patient advocacy skills
