Roles & Responsibilities

• Perform comprehensive initial assessments of clients and families to identify home health needs and establish baseline care plans
• Regularly re-evaluate client nursing needs and update nursing diagnoses based on assessment data
• Initiate and revise individualized plans of care as client status and needs change
• Develop meaningful, goal-oriented care plans that include therapeutic, preventive, and rehabilitative nursing actions with client/family participation
• Administer medications and treatments as prescribed and monitor response
• Provide clear, compassionate health education to clients and families tailored to the plan of care
• Identify and implement discharge planning needs to ensure smooth transitions of care
• Serve as case manager when assigned, coordinating care and communicating changes with physicians and the care team
• Prepare timely clinical notes and update primary physicians at required intervals (at least every 60 days) and as needed
• Ensure equipment and necessary services are available and coordinate with vendors or agencies as required
• Supervise, instruct, and evaluate home health aide care when applicable

Qualifications

• Current Registered Nurse (RN) licensure in good standing (able to practice in North Dakota)
• BLS (Basic Life Support) certification - required
• Home Health and Hospice experience is required
• Minimum of 1 year of relevant nursing experience (per job posting)
• Valid Driver's License and current Auto Insurance required (travel within service area expected)
• Comfortable using HomeCare HomeBase EMR or similar charting systems; strong documentation skills
• Ability to participate in on-call, weekend and holiday rotations
• Willingness to complete required orientation (approximately 12 hours) and any upfront RTO requirements
• Must meet radius/eligibility rules (no full-time/part-time/PRN employment with CommonSpirit-associated facilities including CHI/Dignity within the past year)

Education

• Active RN license (required)
• BLS certification (required)
• BSN not required for submission (per job posting)

Knowledge & Skills

• Skilled in patient assessment, care planning and medication administration
• Experience with case management and coordinating multidisciplinary care
• Proficient charting and clinical documentation practices
• Strong communication skills for physician updates and family education
• Comfortable with pediatric, adult and geriatric patient populations
• Familiarity with HomeCare HomeBase EMR (or similar systems) and general EMR workflows
• Ability to prioritize visits and manage a geographically dispersed caseload
• On-call triage skills and clinical decision-making in home settings
