Shift Type: Monday-Friday, 8:00 AM - 4:30 PM Day (5x8; facility cannot accommodate 4x10s or 3x12s; no required weekend shifts)
Hours per Week: 40
Candidate Pay Rate: $69 per hour

Roles & Responsibilities
- Perform comprehensive admission and start-of-care assessments to establish individualized home health plans
- Deliver skilled nursing care including wound care, medication administration, and IV infusions
- Manage pain and PCA pumps; use pain scales (verbal and nonverbal) to guide interventions
- Complete timely and accurate documentation in OASIS and EPIC for admissions, recertifications, resumption of care, routine visits, and discharges
- Implement infection control precautions and ensure patient and caregiver education for safety at home
- Coordinate care with case management and interdisciplinary partners to support patient goals
- Maintain productivity while providing high-quality visits (may see up to 6 patients/day; 30 productivity points standard)
- Assess and triage patient changes, escalate concerns appropriately, and contribute to safe transitions of care
Qualifications
- Active RN license (Oregon) — pending licenses accepted (please disclose status)
- Minimum 1 year of skilled adult home health experience required — first-time travelers are not accepted
- BCLS certification required
- Proven experience with OASIS and EPIC charting (OASIS experience specifically required)
- Competence with wound care, medication administration (including intradermal, IM, subcutaneous, IV infusions), and PCA pump management
- Reliable transportation (traveler expected to have own vehicle) and ability to travel up to occasional 50+ mile radii
- Facility requires primary series COVID vaccination plus one booster (per facility guideline)

- Home Health Nursing best practices and adult home health care
- Admission assessment, start-of-care, resumption of care, recertification, discharge processes
- OASIS and EPIC documentation standards and computerized documentation
- Wound assessment and evidence-based wound care techniques
- Medication assessment/management and intravenous infusion skills
- Pain assessment and management strategies; PCA pump operation
- Infection control precautions and patient safety in the home environment
- Strong clinical judgment, patient education, and interdisciplinary communication
