Roles & Responsibilities
- Conduct thorough biopsychosocial and psychosocial assessments to guide patient care and discharge planning
- Develop person-centered discharge plans in collaboration with RN case managers and the interdisciplinary team
- Provide brief clinical interventions, crisis intervention, and suicide screening with safety planning as appropriate
- Coordinate care and community resources to ensure safe transitions from the ED to home or other settings
- Document assessments, care plans, and patient interactions clearly and in Epic according to facility standards
- Complete mandated reports of harm per state statutes and liaise with protective services when needed
- Participate in care conferences and refer to internal and external resources to support patient needs
- Support pediatric, adult, and geriatric populations across Emergency Department needs as part of consult-based social work
Qualifications
- LMSW or LCSW required; AK license required (pending AK license accepted)
- Master of Social Work from a CSWE-accredited school
- Minimum 1 year of experience; acute hospital/acute care experience is required
- ER experience preferred and experience with ER care management is a plus
- Ability to float within scope to meet facility needs and provide weekend coverage (every-other-weekend)
- Must be able to submit all RTO restrictions with submittal; local candidates not accepted (candidate permanent address must be at least 50 miles from facility)
- Comfortable working with diverse age groups including pediatrics, adolescents, adults, and geriatrics
- Familiarity with Epic charting is preferred
Education
- Master of Social Work (MSW) from a CSWE-accredited school; LMSW or LCSW licensure in Alaska (pending OK)
Knowledge & Skills
- Psychosocial assessment and biopsychosocial evaluation
- Discharge planning and care coordination with internal and external stakeholders
- Crisis intervention, suicide screening, and safety planning
- Community resource navigation including Medicaid, Medicare, and local assistance programs
- Documentation best practices and clear communication in Epic
- Knowledge of protective services, adult and child protective protocols, and emergency detention
- Familiarity with common major disease processes (CHF, COPD, dementia, diabetes, psychiatric disorders)
- Ability to provide culturally sensitive care and adhere to NASW code of ethics
