Roles & Responsibilities
- Provide skilled nursing care and rehabilitation-focused interventions for patients in the unit
- Perform thorough patient assessments and develop individualized care plans in collaboration with the interdisciplinary team
- Administer medications and treatments safely and document accurately
- Support patient mobility and functional recovery through therapeutic activities and coordination with rehab therapists
- Educate patients and families on care plans, discharge goals, and home safety
- Monitor and escalate changes in patient condition promptly to ensure excellent outcomes
- Contribute to a positive unit culture by mentoring teammates and participating in quality improvement initiatives
Qualifications
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (eligible or active in New Mexico preferred)
- Basic Life Support (BLS) certification required; additional certifications (ACLS, PALS) a plus
- Experience or comfort working in rehabilitation or post-acute nursing settings
- Strong clinical assessment, medication administration, and documentation skills
- Ability to work independently on night shift and collaborate with interdisciplinary teams
Education
- Graduate of an accredited nursing program (ADN, ASN, or BSN) or approved LPN program
Knowledge & Skills
- Solid knowledge of rehabilitation nursing principles and restorative care
- Proficient in medication administration, wound care, and monitoring complex patients
- Skilled in patient education and discharge planning with a focus on functional independence
- Comfortable using electronic medical records and completing timely documentation
- Excellent communication, time management, and critical thinking skills
