Roles & Responsibilities

• Provide skilled nursing care and rehabilitation-focused interventions for patients in the unit
• Perform thorough patient assessments and develop individualized care plans in collaboration with the interdisciplinary team
• Administer medications and treatments safely and document accurately
• Support patient mobility and functional recovery through therapeutic activities and coordination with rehab therapists
• Educate patients and families on care plans, discharge goals, and home safety
• Monitor and escalate changes in patient condition promptly to ensure excellent outcomes
• Contribute to a positive unit culture by mentoring teammates and participating in quality improvement initiatives

Qualifications

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (eligible or active in New Mexico preferred)
• Basic Life Support (BLS) certification required; additional certifications (ACLS, PALS) a plus
• Experience or comfort working in rehabilitation or post-acute nursing settings
• Strong clinical assessment, medication administration, and documentation skills
• Ability to work independently on night shift and collaborate with interdisciplinary teams

Education

• Graduate of an accredited nursing program (ADN, ASN, or BSN) or approved LPN program

Knowledge & Skills

• Solid knowledge of rehabilitation nursing principles and restorative care
• Proficient in medication administration, wound care, and monitoring complex patients
• Skilled in patient education and discharge planning with a focus on functional independence
• Comfortable using electronic medical records and completing timely documentation
• Excellent communication, time management, and critical thinking skills
