Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients with professionalism and warmth
• Administer medications and treatments accurately and safely according to facility policies
• Perform thorough patient assessments and promptly document changes in condition
• Collaborate with the interdisciplinary team to create and follow individualized care plans
• Educate residents and families on care plans, treatments, and health maintenance
• Monitor and maintain accurate clinical records using the facility’s documentation systems
• Maintain a safe, clean environment and follow infection control protocols

Qualifications

• Active Licensed Practical Nurse (LPN) license or LPN eligible (state licensure requirements apply)
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfortable with medication administration, basic wound care, and routine nursing procedures
• Strong communication skills and a team-player attitude
• Previous LPN experience preferred but enthusiastic, caring new grads are welcome to apply

Education

• Vocational diploma or certificate in Practical Nursing (or equivalent)

Knowledge & Skills

• Solid clinical assessment and observation skills
• Proficient documentation and charting practices (electronic health records experience a plus)
• Knowledge of standard infection prevention and safety procedures
• Ability to prioritize tasks in a fast-paced environment
• Empathy, strong interpersonal skills, and the ability to build rapport with patients and families
