Roles & Responsibilities

• Provide compassionate bedside care to patients as a Licensed Practical Nurse
• Administer medications and treatments accurately and safely
• Monitor and document patient vital signs and clinical changes
• Collaborate with the interdisciplinary team to coordinate care plans
• Maintain clear, timely documentation in the patient record
• Educate patients and families about care plans and safety measures
• Follow facility policies, infection control standards, and safety protocols
• Participate in continuous quality improvement and hand-off communications

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania
• Valid Basic Life Support (BLS) certification or willingness to obtain
• Clinical experience in a long-term care or acute care setting preferred
• Strong communication skills and ability to work in a team-based environment
• Ability to pass required background and health screenings

Education

• Graduate of an accredited practical nursing program or equivalent diploma

Knowledge & Skills

• Solid knowledge of medication administration and documentation
• Patient assessment and monitoring skills
• Basic wound care and infection prevention practices
• Proficiency with electronic health records and charting
• Ability to prioritize tasks and respond to changing patient needs
• Strong interpersonal skills for patient and family interactions
