Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents at Hopkins Center
• Administer medications and treatments accurately and safely
• Perform thorough patient assessments and communicate changes promptly
• Document care and interventions clearly in the medical record
• Collaborate with interdisciplinary team members to coordinate patient care
• Support admission, discharge and transfer processes with attention to detail
• Educate residents and families about care plans and promote patient dignity

Qualifications

• Active Licensed Practical Nurse (LPN) credential — required
• Current basic life support (BLS/CPR) certification preferred
• Ability to work 12.5-hour day shifts as scheduled
• Current immunizations and ability to pass required health screenings
• Willingness to complete facility onboarding, background checks and compliance training

Education

• Diploma or certificate from an accredited Practical Nursing program (LPN)

Knowledge & Skills

• Strong medication administration and documentation skills
• Skilled in basic patient assessments and wound care
• Comfortable with electronic charting and following care plans
• Excellent communication and teamwork skills
• Good time management and the ability to prioritize patient needs
