Roles & Responsibilities
- Assess patients' functional mobility and develop individualized treatment plans
- Deliver hands-on physical therapy interventions and therapeutic exercise
- Monitor progress, update care plans, and adjust interventions to meet goals
- Collaborate with nursing, OT, SLP and other team members to coordinate care
- Educate patients and families on safe mobility, fall prevention and home programs
- Document interventions and outcomes accurately in the facility's records
- Support safe discharge planning and communicate recommendations to the care team
Qualifications
- Active Physical Therapist license in Pennsylvania (or ability to obtain prior to start)
- Doctor of Physical Therapy (DPT) or equivalent degree preferred
- New graduates welcome; facility accepts clinicians with any experience level
- Current CPR/BLS certification
- Comfort working in a skilled nursing facility environment and collaborating with a rehab team
- Strong communication skills and solid documentation practices
Education
- Doctor of Physical Therapy (DPT) preferred; equivalent PT degree accepted
Knowledge & Skills
- Skilled in therapeutic exercise, gait training and balance interventions
- Familiarity with skilled nursing facility workflows and Medicare documentation
- Good clinical reasoning for progressions and discharge planning
- Proficient with common rehab equipment and assistive devices
- Team-oriented approach and strong patient/family education skills
- Basic electronic health record documentation experience preferred
