Roles & Responsibilities

• Assess patients' functional mobility and develop individualized treatment plans
• Deliver hands-on physical therapy interventions and therapeutic exercise
• Monitor progress, update care plans, and adjust interventions to meet goals
• Collaborate with nursing, OT, SLP and other team members to coordinate care
• Educate patients and families on safe mobility, fall prevention and home programs
• Document interventions and outcomes accurately in the facility's records
• Support safe discharge planning and communicate recommendations to the care team

Qualifications

• Active Physical Therapist license in Pennsylvania (or ability to obtain prior to start)
• Doctor of Physical Therapy (DPT) or equivalent degree preferred
• New graduates welcome; facility accepts clinicians with any experience level
• Current CPR/BLS certification
• Comfort working in a skilled nursing facility environment and collaborating with a rehab team
• Strong communication skills and solid documentation practices

Education

• Doctor of Physical Therapy (DPT) preferred; equivalent PT degree accepted

Knowledge & Skills

• Skilled in therapeutic exercise, gait training and balance interventions
• Familiarity with skilled nursing facility workflows and Medicare documentation
• Good clinical reasoning for progressions and discharge planning
• Proficient with common rehab equipment and assistive devices
• Team-oriented approach and strong patient/family education skills
• Basic electronic health record documentation experience preferred
