Roles & Responsibilities

• Provide thorough occupational therapy evaluations and develop individualized treatment plans
• Deliver evidence-based interventions to improve daily living skills and functional independence
• Collaborate with nurses, PTs, SLPs, and support staff to coordinate patient-centered care
• Document patient progress accurately and timely in the medical record
• Educate patients and families on strategies, adaptive equipment, and home safety
• Participate in care conferences and contribute to discharge planning for safe transitions
• Maintain compliance with facility policies, regulatory standards, and best practice guidelines

Qualifications

• Active California Occupational Therapy license required
• NBCOT certification preferred
• Current BLS/CPR certification or ability to obtain upon hire
• Ability to pass background check, drug screen, and meet facility immunization requirements
• Comfort working in a post-acute rehab environment with multidisciplinary teams
• Strong communication and documentation skills

Education

• Bachelor or Master of Occupational Therapy (or equivalent) from an accredited program

Knowledge & Skills

• Skilled in ADL assessment, adaptive equipment recommendations, and therapeutic activity design
• Knowledge of post-acute rehabilitation practices and discharge planning
• Proficient documentation practices and ability to use electronic medical records
• Strong clinical reasoning, patient education, and family-centered care approaches
• Ability to prioritize caseloads and manage time effectively in a fast-paced setting
