Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as an LPN, ensuring dignity and comfort
• Administer medications and treatments accurately and timely according to care plans
• Collaborate with RNs, CNAs, and interdisciplinary teams to coordinate excellent patient-centered care
• Document clinical observations, treatments and resident responses clearly and promptly
• Monitor vital signs and condition changes, escalating concerns to the care team when needed
• Educate residents and families about care plans, medications and health maintenance
• Support a positive, safe environment by following infection control and facility policies

Qualifications

• Current, active LPN license in Pennsylvania or eligible to obtain state licensure
• Completion of an accredited practical/vocational nursing program
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Comfortable administering medications, charting, and following established care plans
• Strong communication skills and a collaborative, team-first attitude
• Prior long-term care or skilled nursing experience preferred but not required

Education

• Graduate of an accredited Practical/Vocational Nursing program (LPN)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and monitoring of chronic conditions
• Familiarity with electronic health record charting (facility-specific systems vary)
• Ability to perform resident assessments and report clinical changes
• Infection control procedures and safe patient handling
• Team communication and time management in a busy clinical setting
