Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents at Hillcrest Center
• Administer medications and treatments accurately and according to protocol
• Document assessments, interventions, and patient responses in the medical record
• Collaborate with nurses, providers, and support staff to create clear care plans
• Monitor patients for changes in condition and escalate concerns promptly
• Support residents with activities of daily living while promoting independence
• Participate in shift handoffs and maintain continuity of excellent care

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing (required)
• Basic Life Support (BLS/CPR) certification or ability to obtain upon hire
• Immunizations and health screenings as required by facility policy
• Ability to pass background check and drug screening per facility requirements
• Comfortable working in long-term care/rehab environments and strong teamwork skills
• Previous LPN experience preferred but motivated new grads are welcome to apply

Education

• Completion of an accredited LPN/LVN program or equivalent practical nursing diploma

Knowledge & Skills

• Medication administration, vital signs monitoring, and wound care basics
• Familiarity with electronic medical records and accurate charting
• Strong clinical assessment and critical thinking skills
• Clear, compassionate communication with residents and families
• Time management and the ability to prioritize in a busy setting
• Ability to work collaboratively with interdisciplinary teammates
