Roles & Responsibilities

• Perform thorough admission evaluations to establish baseline function and create individualized care plans
• Deliver skilled treatments focused on improving mobility, strength, and independence
• Document all evaluations, treatments, progress notes, and discharge summaries accurately and timely
• Lead discharge planning to ensure smooth transitions and optimal patient outcomes
• Collaborate with nursing and interdisciplinary team members to coordinate patient care
• Monitor patient progress and adjust treatment plans based on clinical findings and goals

Qualifications

• Valid Physical Therapist license for California (must be able to practice in CA)
• Experience in a Skilled Nursing Facility or long-term care setting preferred
• Strong competency in admission evaluations, treatment planning, progress notes, and discharge documentation
• Current BLS/CPR certification (or willingness to obtain)
• Excellent communication and teamwork skills to collaborate with patients and staff
• Ability to manage a caseload in a 45-bed facility with professionalism and clinical judgment

Education

• Doctor of Physical Therapy (DPT) preferred; Master's or Bachelor's in Physical Therapy considered

Knowledge & Skills

• Skilled in patient evaluation and skilled treatment techniques for older adults
• Knowledge of discharge planning and long-term care best practices
• Proficient documentation skills and familiarity with clinical charting standards
• Understanding of safety, mobility aids, and fall prevention strategies
• Ability to prioritize and manage a varied caseload in a busy SNF environment
