Roles & Responsibilities

• Administer medications accurately following the five rights to keep residents safe
• Assess residents regularly and report any changes in condition to the charge nurse
• Communicate updates to physicians and family members when resident status changes
• Assist CNAs with resident care tasks and respond to call lights as needed
• Document clinical observations and medication administration clearly and promptly
• Report any allegations of abuse, neglect or misappropriation to nursing leadership
• Support a positive, resident-centered environment through compassionate care

Qualifications

• Current Wisconsin LPN license required and must be maintained
• Current CPR/BLS certification required
• 1+ years of related LPN experience preferred, especially in long-term care
• Demonstrated attention to detail and strong clinical documentation skills
• Comfortable communicating with physicians, families, charge nurses and CNAs
• Ability to assist with hands-on resident care and medication administration

Education

• Graduation from an accredited practical nursing program or equivalent training; equivalent combination of education, training and experience considered

Knowledge & Skills

• Medication administration and the 5 Rights
• Resident assessment and change-of-condition recognition
• Clinical reporting and accurate documentation
• Long-term care (LTC) workflows and resident-centered care
• Basic life support and emergency response
• Teamwork skills to work alongside CNAs and nursing leadership
