Roles & Responsibilities

• Provide skilled physical therapy services to residents in a nursing and rehabilitation setting
• Assess functional mobility and create individualized treatment plans to meet patient goals
• Deliver therapeutic interventions such as gait training, balance work, and strengthening
• Document clinical progress accurately and timely in the patient record
• Collaborate with nursing, therapy aides, and families to ensure coordinated care
• Assist with safe transfers, fall prevention strategies, and mobility training
• Participate in care planning and discharge planning to support patient outcomes
• Mentor and support therapy assistants and aides to enhance team performance

Qualifications

• Current, unrestricted Physical Therapist license (or eligible) for Montana
• Graduation from an accredited physical therapy program
• Basic Life Support (BLS/CPR) certification or ability to obtain upon hire
• Experience in skilled nursing or rehabilitation settings preferred
• Strong documentation and clinical reasoning skills
• Ability to pass required background checks and health screenings

Education

• Graduation from an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, and transfers
• Knowledge of geriatric and post-acute care needs
• Proficient clinical documentation and outcome measurement
• Strong communication and interdisciplinary teamwork skills
• Ability to adapt treatment plans to individual resident needs
