Roles & Responsibilities

• Provide therapeutic procedures under the supervising therapist's plan to help residents regain functional abilities and independence
• Follow the treatment plan of care, monitor resident progress, and modify approaches after consulting with the evaluating therapist
• Document all therapeutic interactions promptly and accurately so the whole care team stays informed
• Educate residents and caregivers on exercises, assistive device use, and safety to promote lasting outcomes
• Recommend, fit, and instruct on assistive devices, ensuring safe and confident use
• Collaborate in interdisciplinary team meetings and communicate changes in resident status immediately
• Maintain clinic and equipment cleanliness and follow infection control procedures to protect residents and staff
• Support the development of new rehabilitation programs and pursue learning to enhance resident care

Qualifications

• Associate degree in Physical Therapy Assistant or related field (required)
• Licensed Physical Therapist Assistant (state PTA license required)
• Basic Life Support (BLS) certification required
• Prior experience in long-term care or a skilled nursing facility preferred
• Ability to safely lift up to 50 pounds (requires two-person lift above 50 lbs)
• Strong documentation, verbal and written communication skills
• Proficiency with Microsoft Office for documentation and reporting

Education

• Associate degree in Physical Therapy Assistant or related field (required)

• Therapeutic procedures and physical therapy modalities
• Treatment planning, implementation, and progression toward functional goals
• Patient and caregiver education and presentation skills
• Accurate and timely documentation practices
