Roles & Responsibilities

• Develop and periodically review individualized care plans in collaboration with physicians, patients, and caregivers
• Coordinate the patient care plan through clear interdisciplinary communication and case conferences
• Perform ongoing clinical, functional, and emotional assessments to identify problems or progress toward goals
• Document evaluations, treatment goals, and objectives thoroughly and timely
• Communicate significant clinical changes to the patient’s physician and VNA colleagues promptly
• Provide patient and caregiver education on self-care, medications, diet, and health counseling

Qualifications

• Current, active Registered Nurse license in Vermont and New Hampshire OR an unencumbered compact RN license
• Graduate of an accredited school of nursing
• Minimum 2 years of home health experience

• Graduate of an accredited school of nursing

• Home health nursing and RN case management
• Clinical assessment and care planning
• Patient education and self-care coaching
• Accurate documentation and record-keeping
• Care coordination and interdisciplinary communication
• Medication management, hospice, and rehabilitation familiarity
