Roles & Responsibilities

• Provide skilled nursing care during in-home visits, delivering compassionate home health and hospice services
• Perform comprehensive patient assessments and develop individualized care plans
• Document visits and patient progress accurately using HCHB
• Coordinate care with therapy teams, unit staff, and hospice resources to ensure continuity
• Educate patients and caregivers on treatments, medications, and safety plans
• Manage a daily caseload (approximately 5-6 patients per day) efficiently and safely
• Adhere to agency policies and maintain high standards of patient experience and compliance

Qualifications

• Active Registered Nurse (RN) license in North Carolina
• Minimum 2 years of nursing experience, with home health or hospice experience required
• Experience using HCHB (Homecare Homebase) for documentation preferred
• Reliable vehicle and valid driver’s license required for patient visits
• Strong clinical assessment, time management, and patient education skills

Education

• Active RN license (required)
• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Proven home health and hospice clinical knowledge
• Proficient with HCHB documentation workflows
• Skilled in patient assessments, wound care basics, and medication management
• Excellent communication and multidisciplinary collaboration
• Comfortable working independently during home visits and managing a caseload
