Roles & Responsibilities
- Assess patient needs in the home setting and create individualized care plans
- Provide skilled nursing care including wound care, medication management, and symptom control
- Deliver compassionate hospice support when appropriate, honoring patient goals
- Coordinate care with therapists and interdisciplinary team members to ensure continuity
- Educate patients and families to empower them in managing conditions at home
- Document thoroughly and accurately in the patient record and follow agency policies
- Advocate for patient needs and communicate changes promptly to the care team
Qualifications
- Current RN license in North Carolina (required)
- Home health or hospice nursing experience, serving Franklin/Macon County patients
- Basic Life Support (BLS) certification required; hospice/home health certifications preferred
- Comfortable working independently in patients' homes and managing caseloads
- Valid driver’s license and reliable transportation for home visits
Education
- Associate Degree in Nursing (ADN) required, BSN preferred
Knowledge & Skills
- Strong clinical assessment and critical thinking skills
- Experience with home health documentation and care planning
- Excellent communication and patient/family education skills
- Ability to collaborate with therapists and interdisciplinary teams
- Time management and organizational skills for field-based work
