Roles & Responsibilities

• Evaluate residents using standardized assessments and clinical judgment to identify functional needs
• Develop individualized treatment plans with clear short- and long-term goals focused on ADLs and independence
• Deliver therapeutic interventions and modalities safely and effectively, monitoring progress toward goals
• Document all evaluations, treatments, and education in the Electronic Medical Record so the whole team stays informed
• Recommend and instruct residents and caregivers on appropriate assistive devices and safe use
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate discharge planning and ongoing care
• Supervise and mentor assistant-level staff, ensuring care meets state and professional standards
• Provide in-service education and contribute to program development to improve rehabilitation services
• Maintain infection-control practices and keep clinical spaces and equipment clean and ready for care

Qualifications

• Active PT license in Massachusetts (must hold or be eligible to obtain prior to start)
• Bachelor's degree required; Master’s or Doctorate in Physical Therapy preferred
• Basic Life Support (BLS) certification preferred or must obtain promptly
• Prior experience in long-term care or skilled nursing preferred
• Familiarity with Medicare, Medicaid and documentation/reimbursement requirements
• Proficiency with Electronic Medical Record systems and solid documentation skills
• Strong verbal and written communication, time management, and teamwork skills
• Ability to lift and assist residents safely (able to lift up to 50 pounds; above 50 requires two-person lift)

Education

• Bachelor's degree required
• Master's or Doctorate in Physical Therapy preferred

Knowledge & Skills

• Patient evaluation and treatment plan development
• Therapeutic modalities and ADL-focused interventions
• Assistive device recommendation and patient/caregiver education
• Electronic Medical Record (EMR) proficiency
• Knowledge of Medicare/Medicaid and reimbursement-related documentation
• Microsoft Office basics and presentation skills
• Interdisciplinary team collaboration and supervision of assistant-level staff
• Strong interpersonal, communication, and time-management abilities
