Roles & Responsibilities
- Provide high-quality physical therapy services as a PTA in a skilled nursing facility, helping residents regain mobility and independence
- Implement treatment plans developed by supervising PTs and document patient progress thoroughly
- Collaborate with nursing and interdisciplinary teams to coordinate care and improve patient outcomes
- Meet facility productivity expectations and contribute to efficient, patient-centered therapy workflows
- Follow Continuum Rehabilitation Group policies and procedures to maintain safety and compliance
- Represent the facility professionally, adhering to the black-scrubs dress code and onsite presence requirements
Qualifications
- Licensed Physical Therapist Assistant (PTA) in good standing (state licensure required)
- 1-3 years of PTA experience (Skilled Nursing Facility experience strongly aligned with this role)
- Comfortable following CRG policies & procedures and meeting productivity expectations (92% productivity noted)
- Ability to be onsite to receive guaranteed hours and maintain reliable attendance
- Strong interpersonal skills and ability to collaborate within an interdisciplinary care team
Education
- Graduation from an accredited PTA program (Associate degree or certificate) or equivalent clinical training
Knowledge & Skills
- Skilled in implementing rehabilitation plans and therapeutic exercises for geriatric/SNF populations
- Competent documentation and charting practices for skilled nursing settings
- Good patient assessment and communication skills to engage residents and families
- Familiarity with safety, infection control, and mobility-assist techniques used in SNFs
- Ability to manage caseloads efficiently while maintaining high-quality care
