Roles & Responsibilities

• Perform thorough patient assessments and develop individualized plans of care
• Deliver evidence-based acute care and acute rehab treatments to improve function
• Document evaluations, daily notes, reassessments and discharge summaries in Epic
• Collaborate with nursing, respiratory, social work and pharmacy to coordinate care
• Assist with safe patient mobilization including Hoyer lifts, gait belt use, and transfers
• Provide patient and family education to support recovery and discharge planning
• Monitor vital signs and adjust treatment plans for medically complex patients
• Participate in interdisciplinary rounds and contribute to quality improvement

Qualifications

• Current State Physical Therapy license (State licensed required)
• Minimum 3 years of professional experience (with at least 1 year acute care hospital experience preferred)
• BLS certification required
• Proficiency with Epic charting is required
• Comfort and willingness to float between like units with orientation provided
• Experience with acute rehab, mobility training, Hoyer lifts, and wheelchair assessments
• Ability to work the assigned weekend schedule as noted

Knowledge & Skills

• Assessment, documentation, plan of care, reassessments and discharge planning
• Vital sign monitoring and medically complex patient management
• Lifting/transfer techniques, Hoyer lift operation, gait belt and wheelchair assessment
• Orthopedics, neurology, cardiopulmonary and prosthetic training fundamentals
• Modalities familiarity (cryotherapy, moist heat) and musculoskeletal techniques (McKenzie, myofascial release)
• Strong time management, prioritization, and interdisciplinary communication skills
• Experience in acute care and acute rehab settings; comfortable with varied caseloads
