Roles & Responsibilities
- Provide skilled occupational therapy assessments and treatments in patients' homes, focusing on functional independence and safety
- Complete OASIS documentation and chart in EPIC Remote and Rover accurately and timely
- Develop and implement individualized plans of care based on thorough evaluation and measurable goals
- Perform home assessments and recommend adaptive equipment, home modifications, and safety strategies
- Deliver patient and family education to support safe discharge and community re-entry
- Collaborate with interdisciplinary team members and communicate care plans and progress
- Manage a productive caseload (may visit up to ~6 patients/day) while maintaining quality documentation
- Use clinical judgment to reassess, update POCs, and coordinate referrals as needed
Qualifications
- Active OT licensure in the state required (pending licenses accepted — confirm with hiring manager)
- Minimum 1 year of skilled home health experience required
- BCLS (BLS) certification required
- Demonstrated OASIS experience and comfort charting in EPIC and using Rover mobile app
- Reliable transportation and willingness to travel within approximately a 60-mile radius for visits
- Ability to manage up to six visits per day and meet productivity expectations
- Strong patient/family education, documentation, and care-planning skills
Knowledge & Skills
- OASIS documentation and Home Health processes
- EPIC charting and Rover mobile documentation
- Assessment, evaluation, plan of care, and reassessments for ADLs
- Home assessment and safety recommendation skills
- Adaptive equipment fitting and wheelchair evaluation/positioning
- Splinting skills (dynamic, functional, static) and prosthetics/orthotics familiarity
- Myofascial release, joint mobilization, and other manual therapy techniques
- Cognitive assessment, community re-entry strategies, and crisis intervention
- Familiarity with billing/documentation expectations and National Patient Safety Goals
