Roles & Responsibilities

• Provide compassionate, patient-centered nursing care and basic clinical services
• Administer medications and document care accurately according to policy
• Perform routine assessments and report changes in patient condition promptly
• Assist with activities of daily living while promoting patient dignity and independence
• Collaborate with RNs and interdisciplinary team members to support care plans
• Maintain accurate electronic and paper documentation with attention to detail
• Follow infection control standards and facility procedures to keep patients safe

Qualifications

• Current, active LPN license in Pennsylvania (required)
• Completion of an accredited Practical Nursing program
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Prior LPN clinical experience preferred, especially in long-term care or skilled nursing
• Good communication skills and ability to work well on a team
• Immunizations and background checks completed per facility policy

Education

• Graduate of an accredited Practical Nursing (LPN) program
• Active LPN license in Pennsylvania
• BLS (CPR) certification

Knowledge & Skills

• Medication administration and basic IV/PO knowledge
• Patient assessment and vital signs monitoring
• Charting and documentation best practices (EHR experience a plus)
• Infection prevention and safety procedures
• Time management and prioritization in a busy care setting
• Strong interpersonal skills and professional bedside manner
