Roles & Responsibilities

• Administer medications and treatments safely and accurately according to care plans
• Monitor and document patient vital signs, responses, and progress with attention to detail
• Provide compassionate bedside care and assist with activities of daily living
• Communicate effectively with patients, families, and the interdisciplinary care team
• Maintain timely, accurate documentation in the patient record to support continuity of care
• Collaborate with RNs and facility leadership to escalate concerns and improve patient outcomes
• Support a positive, patient-centered environment and uphold facility policies

Qualifications

• Active Licensed Practical Nurse (LPN) license — New Mexico preferred or required for practice
• Must be local to the Santa Fe area (Local ONLY — no travel assignments)
• Comfortable working the posted schedule: Monday, Tuesday, Wednesday, 6:00 AM - 6:30 PM
• Strong communication and teamwork skills to coordinate care in a fast-paced setting
• Ability to complete facility-required onboarding and state-mandated training

Education

• Graduate of an accredited Practical Nursing program or equivalent vocational training

• Medication administration and safe documentation practices
• Basic patient assessment and vital sign monitoring
• Clinical documentation and charting accuracy
• Patient-centered communication and family engagement
• Time management and prioritization during busy shifts
• Teamwork and collaboration within interdisciplinary care teams
